What is Practice Integration?

PCCN’s Practice Integration Team works directly with our participating network providers. This team develops meaningful relationships with network administrators and physician owners, meeting monthly with each practice.  This interaction assists with promoting quality by providing resources, best practices, training, and education.  Monthly participation ensures Network alignment in value-based goals, adherence to quality measures, and Network information.  We use Practice feedback to improve our clinically integrated program to meet needs and anticipate trends in healthcare. 

PCCN’s Quality Improvement Initiatives 

In pursuit of the Quadruple Aim of Clinically Integrated Networks, PCCN’s Practice Integration Program offers Quality Improvement Initiatives to address many of the key quality health and wellness issues that our community and our providers are faced with. In this program, we offer these initiatives to provide physicians with key pieces of information that they need to address each of these areas. PCCN’s current quality improvement initiatives include:

  • Well Child Visits (WCV)
  • Asthma Management
  • Well-Child Visits in the First 30 Months of Life (W30)
  • Social Determinants of Health (SDoH)
