Our CARE program is part of the Division of Endocrinology and Diabetes. We provide health education, promote the prevention as well as management of premature heart disease, type 2 diabetes and other metabolic dysfunction. Children receive comprehensive disease management resources including medical and lifestyle assessments, nutritional counseling, diabetes education and more.
Our CARE Program is made up of a multidisciplinary team of pediatric specialists who work together to provide comprehensive care for children and adolescents with obesity, pre-diabetes, type 2 diabetes and other metabolic dysfunction.
We use a team approach to provide the best family-centered care. Our team includes:
- Pediatric endocrinologists
- Pediatric registered dietitians (RD)
- Certified diabetes care and education specialists (CDCES)
- Pediatric behavioral health specialist
- Your child’s primary healthcare provider
- Other pediatric specialists (sleep, liver, high blood pressure, women’s health) as needed
The CARE program generally sees patients with a body mass index (BMI) of greater than 85th percentile and who also have one of the following criteria:
- A1c 5.7% or above (>6.5% please mark as urgent)
- Fasting glucose 100 or above; or non-fasting glucose >140
- Fasting triglycerides 130 or above
- Fasting LDL greater than 130
- Non-HDL >145
Referrals can be marked as CARE program and faxed to 602-933-2436.
