Phoenix Children’s Center for Fetal and Neonatal Care offers a unique, concierge-style resource for families. As you prepare to leave the NICU and enter the next phase of caring for your child, our team is here to help coordinate the many services, providers and support resources your family will need.

Life After the NICU

ur dedicated nurse coordinators support families of children who require care coordination with multiple specialists for complex medical conditions. They can also help with the transition from inpatient to outpatient care. Often, this team connects with families while still in the NICU to begin providing support immediately at discharge. 

We know that navigating and coordinating the many services required for newborns with complex conditions can be overwhelming, especially at first. We offer this service to make the process easier for families by: 

  • Coordinating services, providers and support resources needed 
  • Answering questions 
  • Helping families identify and reach specialists 
  • Making appointments and referrals 
  • Getting insurance authorizations
  • Managing home health companies And more! 

Our team is available to these families throughout their child’s early years to ensure that each child receives the best start possible. During that time, we will also connect families with other services and programs that can help, like our Newborn Early Screening Team (NEST) and other condition-specific clinics. 

Who is eligible?

This service is available to our NICU families, including families at our partner NICUs with Dignity Health. In fact, the team often coordinates telehealth NICU discharge appointments as part of the initial support provided to families.
