Leaving the neonatal intensive care unit (NICU) can feel overwhelming, but you’re not alone. The Center for Fetal and Neonatal Care offers a unique, personal resource for families. We’ll help guide and coordinate the services, specialists and support you and your baby need as you take the next steps.

Your Personal Care Coordinators

If your baby has a complex medical condition, your dedicated nurse coordinator will help you navigate care with multiple specialists for all their needs. They’ll be by your side during the transition from hospital to home. In many cases, they’ll connect with your family while your baby is still in the NICU, so you start getting support even before you leave.

We know it can be tough to manage all the appointments and services for your newborn, especially when everything feels so new.

Our goal is to lighten the load by helping you:

  • Get answers to your questions
  • Contact insurance companies to obtain authorizations
  • Coordinate services, providers and support resources
  • Identify and reach specialists at Phoenix Children’s and in the community
  • Make appointments and referrals
  • Work with home health companies for home equipment


Our team stays by your side throughout your child’s early years to give them the strongest start possible. Along the way, we’ll connect your family with any other services or programs that might help, like our Newborn Early Screening Team (NEST) and other condition-specific clinics.

Telehealth Neonatal Program

Our Telehealth Neonatal Program connects families and their chosen pediatricians with our neonatology team before discharge. Through secure video visits, we review your baby’s hospital course, discuss care plans and ensure a smooth handoff. This way, your pediatrician can ask questions and help you feel more confident about the next step in your baby’s care.
