Medical Record Requests

To request patient medical records, please contact our medical record department Monday through Friday between 8 a.m. to 4:30 p.m. If your child is currently a patient at Phoenix Children’s, you can access their medical records through the Patient Portal.

Immunization Record Requests

To request a copy of your child’s immunization records, please use the Phoenix Children’s Patient Portal. You can also contact our Health Information Management Services (HIMS) team or check the Arizona Department of Health Services immunization registry.
