How to Prepare for Your First Appointment

Thank you for choosing Phoenix Children’s Pediatrics for your child’s healthcare needs. We look forward to providing the best possible care and service. To help you get started, here’s what to expect from the new patient process:

1. Families should verify that Phoenix Children’s Pediatrics accepts their insurance plan.

2. Contact the Phoenix Children’s Pediatrics location most convenient for you and schedule your child’s first appointment. You will receive a text message with a link to preregister online.

3. Bring the following documents to your child’s first appointment:

  • Photo ID
  • Insurance card
  • Complete list of your child’s current medications (including over-the-counter medicines)
  • A list of questions to review with the healthcare provider
  • Your child’s current immunization records (if applicable)

At Phoenix Children’s Pediatrics, we believe vaccines play an important role in protecting children, families and our community from serious illnesses. Our clinics welcome families who follow the vaccine schedule recommended by the American Academy of Pediatrics.

Mobile Registration

Phoenix Children’s Pediatrics offers mobile registration so you can check in online from your own device before your appointment. You can also check in on your own device when you arrive. Ask us about mobile registration and watch for an email or text before your next appointment.  

Patient Rights and Privacy

Your family’s privacy and rights are important to us. We encourage you to review the Phoenix Children’s Notice of Privacy Practices, Health Information Exchange (HIE) and Patient & Family Rights forms before your first visit.
