Steps you can take for continuity of care if you have BlueCross BlueShield

Your child’s care is important to us, and we want to continue to provide the high-quality health care you’ve come to expect from Phoenix Children’s and our pediatric experts.

Phoenix Children’s is no longer a contracted provider with BlueCross BlueShield as of October 31, 2024. The care you currently receive from Phoenix Children’s, as well as our physician providers and urgent care centers, is considered out-of-network and you may see changes in your financial responsibility for services at Phoenix Children’s.

Here are steps you can take before your next appointment:

  1. Call the number listed on the back of your BlueCross BlueShield insurance card to learn about your out-of-network benefits under your plan.
  2. Apply for continuity of care before your next appointment. If you have BCBS of Arizona, you can use this form. If you have another BCBS plan, contact the number on the back of your insurance card to request the form specific to your plan.
  3. During open enrollment, explore the many plans that are in network with Phoenix Children’s.

NOTE: Emergency care access is not impacted. You may continue to receive emergency care at our three Emergency Room locations in Phoenix, Avondale and Glendale, regardless of our network status with BlueCross BlueShield.
