Learning from Patient Harm: A Case-based Presentation on Medical Errors and Patient Safety
This session will review events in which system errors occurred and resulted in harm to a patient.
At the end of this presentation, participants should be able to:
Objectives:
- Review and discuss a patient safety event or known complication that resulted in preventable harm to a patient
- Review action plans put in place in response to the serious safety to decrease the likelihood of such events from occurring again
- Review current literature regarding types of errors that can occur in our health system and encourage care providers to speak out if they believe there is a potential error occurring
- Encourage a culture of transparency and improvement in an ongoing commitment to zero preventable harm
